Referrals can be made by your county case manager, Vocational Rehabilitation Services (VRS), or self-refer for some programs. Please reach out if you are unsure and one of ODC’s dedicated staff will assist you in the process.

What information gets collected with an application?

A thorough assessment is completed prior to admission. Information collected may include, but not limited to: medical diagnosis, medication, physical, cognitive, social, emotional and recreational needs and wants, authorization for funding, and individual and family information.

Who should make the initial inquiry for ODC services?

An inquiry or referral is usually initiated by families or care managers.

What if there is bad weather?

Day and employment services may cancel services in the case of extreme weather, all persons served will be notified if this happens.
